SUBMISSION OF PROPOSAL

1. Completed proposal documents must be submitted in hard copy and soft copy form as follows:

• • a. 1 hard copy of the original set of proposal containing the name of the Bidder, clearly marked as “ORIGINAL”;

• • b. 4 hard copies of the set of proposal without the name of the Bidder, clearly marked as “COPY”;

• • c. 1 USB Flash Drive containing a soft copy of the original set of proposal containing the name of the Bidder, clearly marked as “ORIGINAL”; and

• • d. 1 USB Flash Drive containing a soft copy of the set of proposal without the name of the Bidder, clearly marked as “COPY”.

2. The USB Flash Drives should be submitted in a separate envelope and clearly labeled as “SOFT COPY” on the top left of the envelope.
